Resident Care Coordinator

Concordia at SpiriTrust Kelly Drive is seeking a compassionate and motivated Resident Care Coordinator to oversee and coordinate quality resident care within our Personal Care community. This leadership role is responsible for supervising nursing staff, ensuring compliance with state regulations, maintaining accurate documentation, and promoting a positive environment for residents, families, and team members. Our Resident Care Coordinator is a full-time, first shift position.

Responsibilities:

• Coordinate and oversee resident care services
• Supervise nursing staff and provide leadership/support
• Communicate with physicians, families, consultants, and interdisciplinary teams
• Complete chart, cart, and room audits
• Ensure compliance with DHS regulations and facility policies
• Monitor resident conditions and report significant changes
• Maintain accurate and timely documentation
• Assist with staff development and performance management

Qualifications:

• Current Pennsylvania LPN or RN license
• Strong leadership and communication skills
• Compassionate, resident-focused approach to care
• Ability to work collaboratively in a team environment

Physical Requirements:

• Ability to lift/move up to 50 pounds
• Ability to stand, walk, bend, and assist residents as needed
