- Interviews patient and family members to obtain information about home environment, family relationships, health history, and patient’s personality traits.
- Evaluates data gathered in terms of the medical plan of treatment, available social service programs, and plan a pertinent therapy program that will provide maximum benefits for each patient.
- Assists patients and families to understand, accept, and follow medical recommendations.
- Performs a variety of services such as advising on social problems, arranging for discharge care at home or in institutions, placement of patients into foster homes or temporary placement, and alleviation of anxieties and fears.
- Utilizes resources to assist patient to resume life in the community or to learn to live with a disability.
- Prepares required documentation of each case. Maintain records, reports and other duties as required.
- Maintains department policies, procedures, objectives, and performance improvement program, safety, infection and environmental control.
- Serves as the social work representative on multi-disciplinary team approach to patient care.
