Social Worker

Prestigious Nursing & Rehab Center in The Bronx (NY) is actively a full-time Social Worker.

Duties Include:

· Participate in reviewing and setting policies concerning resident care and quality of life

· Identifying individual social and emotional needs

· Interpret social, psychological, and emotional needs of the resident, and family to the medical staff, attending physician, and other resident care team members.

· Assist in providing solutions for social and practical environmental problems including seeking financial assistance, discharge planning (including collaboration with community agencies), and referrals to other community agencies when specialized assistance is required

· Developing and maintaining individualized care plans

· Participating in the planning of the resident’s admission, community re-entry or transfer to another facility

· Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the residents

Requirements:

· Must be a New York LCW LCSW, or LICSW

· Must have a working understanding of social systems along with the ability to implement pertinent state and federal regulations

· Must have prior LTC experience
