Contract Duration: 13 Weeks

Job Description

We are seeking a compassionate and experienced Medical Social Worker (MSW) for a 13-week assignment in Napa, California. The ideal candidate will provide psychosocial support, care coordination, and discharge planning services while collaborating closely with interdisciplinary healthcare teams in a clinical setting.

Responsibilities

• Assess patients’ psychosocial needs and develop appropriate care plans
• Provide counseling, emotional support, and crisis intervention services
• Coordinate discharge planning and community resource referrals
• Collaborate with physicians, nurses, case managers, and other healthcare professionals
• Advocate for patients and families to ensure continuity of care
• Maintain accurate and timely documentation

Requirements

• Master’s Degree in Social Work (MSW) required
• Previous healthcare or hospital social work experience preferred
• Strong communication and interpersonal skills
• Ability to manage multiple cases in a fast-paced environment
• Knowledge of discharge planning and community resources preferred
