Salary: $30 - $35 hour DOE
About the Position
Serves as a point of contact for Members. Successfully directs the assessment of members’ needs for services and the delivery of appropriate services. Achieves high member satisfaction in the Goodwin Living At Home program.
Job Duties
- In conjunction with the Director of Member Services, develops a network of subcontracted service providers. Participates in interviewing and selection of service providers, including home care agencies, transportation providers, Home Health Care, Hospice providers, etc. necessary to ensure exceptional delivery of services.
- Obtains and reviews required documents from prospective service providers to ensure they meet Goodwin House at Home standards.
- Develops a network of vendor partners for referrals for non-covered services. Maintains the network and manages the referral process.
- Schedules, conducts, and documents pre-admission assessments of prospective members. Serves on the Care Coordination Team providing input on decisions made regarding applicants and members.
- Conducts new member meetings. Ensures that new members are welcomed into the program quickly and efficiently.
- Communicates regularly with members (minimum of one visit per year, quarterly phone calls and as needed).
- Conducts annual, OT assessments, and as-needed assessments of members to determine the need for services.
- Visits members in rehab centers or at home to determine the need for services and to provide reassurance.
- Manages the successful placement of appropriate services to address members’ needs. Communicates with members regarding what Goodwin Living at Home will pay for (what is ‘covered’), and members’ financial responsibilities.
- Follows up with members to determine their satisfaction with services.
- In conjunction with the Care Coordination Team, manages any quality monitoring initiatives.
- Participates in informational seminars when needed.
- Responsible for networking activities within the market area designed to enhance community awareness of the continuing care at home program.
- Utilizes AOD to document interactions with members, family members or other member associates, manage contacts, member follow-up, etc.
- Participates in the on-call schedule evenings and weekends.
- Provides support and collaborates in the planning, development, implementation, analysis, and documentation of a Goodwin Living program.
- Other tasks as assigned.
Job Requirements
- Understanding of older adults and the challenges of aging in place in one’s home in the community.
- Ability to establish and maintain positive relationships with customers, supervisors, and peers.
- Experience serving older adults a plus, as well as providing services to individuals living in their own homes.
- Minimum of bachelor’s degree, social work degree preferred, but not required.
- Must be fully committed to the mission of Goodwin House Incorporated.
- Ability to work well under stress.
- Completes assignments accurately, works independently and manages time effectively.
- Must exhibit a high degree of initiative and creativity, good judgment, problem solving ability and professional ethics.
- Experience conducting assessments such as the Montreal Cognitive Assessment (MOCA), MMSE, Geriatric Depression Screen, Falls screen etc.
- Must be familiar with Windows operating systems, MS Office and email software.
- Must be willing to work remotely and/or share office space when necessary.
- Excellent verbal and written communication skills.
- A strong commitment to quality care and service and a sensitivity to the needs and concerns of older adults.
- Must be able to work flexible hours including rotating the after-hours on call schedule and taking care of member's needs nights and weekends, as required
