Social Worker - Case Management -Chinese or Farsi language is preferred
Grace Adult Health Center in Santa Clara has openings for a BSW and/or a MSW with case management and geriatric experience. Chinese Mandarin or Farsi Language skill is desired.

RESPONSIBILITIES:

• Provide initial and ongoing psychosocial assessments.
• Develop and implement a comprehensive plan of care based on the Participant’s needs and goals,
• Assist participants and their families to solve medical social problems that arise as a result of the participant’s medical condition or disability.
• Conduct a home visit at initial assessment and as needed depending on the Participant’s need (Not needed currently for Telehealth services)
• Serves as a liaison with the participant’s home and family, and with other community agencies that may be providing services to the participant; coordinate all services delivered to the participant to meet their needs.
• Facilitates group discussions.
• Maintain effective communication, collaborate and coordinate with the multidisciplinary team to coordinate plans of care, to ensure continuity of care, and to maintain positive working relationships.
• Works closely with the Grace ADHC therapist, and monitoring of participants that are able to benefit from group therapy.
• Provide and assist Participants in accessing community resources.
• Maintain records and documentation as required by the department and professional standards.
• All other reasonably related duties as assigned.

QUALIFICATIONS:

BS or MS degree in social work from an accredited school of social work is required. Recent case management experience with the geriatric population is preferred.

Must have excellent oral and written skills in order to prepare and present complete and concise written materials. Prefer to have Chinese Mandarin or Farsi language skills.

Effective time management skills. MUST be skilled in using Microsoft office software with good general computer and networking knowledge.

Please note the LCSW or LMFT applicants can also choose to be part-time consultants.

Pay: $75,000.00 - $96,000.00 per year
