STATEMENT OF PURPOSE
The Social Worker encompasses all functions within the College Hospital Crisis Stabilization Unit (CSU)and is directly accountable for social services and discharge planning. The social worker is accountable for the coordination of patient information between referral sources, family, outside agencies and the treatment team.

QUALIFICATIONS
Master’s Degree in social work or related field with social work training and experience. Maturity, flexibility, leadership qualities, professionalism, and strong clinical skills required.

PHYSICAL DEMANDS
Frequent walking, standing, bending and reaching. Lifting up to 25 pounds. Pulling and pushing up to 50 pounds. Must be able to participate in takedowns. Corrected vision and hearing to a normal range. Finger dexterity. Frequent exposure to stressful situations. Constant public and patient contact.

MAJOR TASKS, DUTIES AND RESPONSIBILITIES
(All duties listed below are essential to the job.)
*General Staff Accountabilities

  • Employee’s conduct conforms to the Mission, Vision, Values, and Code of Conduct of College Hospital Crisis Stabilization Unit (CSU).
  • Values individual differences and demonstrates sensitivity to the cultural needs of others.
  • Respects the rights, privacy and property of others, and maintains strict confidentiality.
  • Effectively protects all health information from unauthorized access per HIPAA regulations and all applicable local and state laws.
  • Maintains required certification/license in a timely fashion.
  • Exhibits good attendance and punctuality.
  • Provides appropriate notice prior to vacation/time off and notifies supervisor a minimum of two hours prior to shift when unable to come to work.
  • Observes allotted break and meal periods as prescribed by College Hospital Crisis Stabilization Unit (CSU)Policy and follows time card procedures by accurately writing in and out.
  • Attends to personal affairs to avoid any interference with productivity.
  • Adheres to the Dress Code.
  • Demonstrates an understanding of their personal role in the case of fire/disaster and participates in Performance Improvement activities.
  • Functions with an awareness of patient safety issues and applies basic principles of safety as identified within the facility.
  • Follows the occurrence reporting policy and procedure in reporting any potential safety issues

*Job Specific Duties

  • Demonstrates the ability to complete psycho social within one hour of client admitting to the CSU
  • Demonstrate the ability to complete an ISH on the same day client admits
  • Demonstrates the ability to communicate with outside resources and identify appropriate placement for each client
  • Demonstrates understanding of CSU’s program; solution focused care and completes the client safety plan with them prior to discharge
  • Demonstrates the ability to chart legibly and accurately in the clients chart at least every four hours or when necessary
  • Demonstrates the ability to recognize clients that are escalating and use de-escalation techniques to avoid the use of seclusion and restraints.
  • Assists clinical team by spending time on the floor
  • Demonstrate the ability to make follow up calls to discharged clients, document as such
  • Demonstrate the ability to use critical thinking in daily situations
  • Competent to assess patient’s needs based on cultural background and functional limitations/disabilities.
  • Demonstrate the ability to make discharge packets, documenting where they were sent and who was spoken too
  • Demonstrates ability to follow all fire and safety guidelines and knows role in case of
  • emergency/disaster
  • Understands and can respond to patient’s cultural, spiritual or functional needs by using appropriate resources, i.e., interpreter list.
  • Demonstrates ability to document admit and discharge planning notes appropriately.
  • Is able to efficiently coordinate discharge of patients to appropriate level of care.
  • Demonstrates the ability to recognize and intervene when clients are decompensating
  • Demonstrates ability to therapeutically intervene with patients and their family/caregiver.
  • Demonstrates knowledge of Infection Control, Standard Precautions, and protecting against
  • blood-borne pathogens, isolation technique, PPE and red bagging, proper use of mask to prevent the spread of airborne pathogens
  • Demonstrates and consistently practices proper handwashing techniques in order to help control the spread of infection
  • Communicate discharge issues with the charge RN
  • Able to document treatment plan based on assessment of patient.
  • Understands and is able to conform to mandated reporting guidelines regarding Child Abuse/Neglect, Elder/Dependent Abuse/Neglect, Domestic Violence and Tarasoff notification.
  • Understands and is able to conform to laws pertaining to patient’s rights.
  • Demonstrates ability to provide and document relevant and timely education to patients and their family/caregiver.
  • Demonstrates excellent Customer Service skills and sensitivity to cultural diversity when dealing with co-workers, patients, physicians and outside agencies.
  • Demonstrates understanding of break and sick call policy and adheres to taking breaks as assigned
  • Performs all other duties as assigned.
