Key Responsibilities:

Ensure accurate and timely documentation in compliance with state and federal regulations
Participate in interdisciplinary care plan meetings to support residents’ social and emotional well-being
Educate residents and families regarding Advanced Directives
Coordinate and facilitate smooth admissions and discharges
Support residents and families through transition planning and ongoing care needs

Qualifications:

Current New Jersey LSW, CSW, or LCSW required
Prior staff management experience preferred
Long-term care experience a plus
Strong communication and organizational skills
Ability to work both independently and collaboratively within an interdisciplinary team
