Description:

• This position involves providing comprehensive care management and quality assurance for participants in a healthcare setting.
• The role requires coordinating holistic care plans sensitive to diverse backgrounds and needs.
• The position is onsite in Bronx, NY, with a contract employment type and standard weekday hours.

Requirements:

• Master's degree in Social Work from an accredited program.
• Minimum of two years of administrative experience in a management capacity in healthcare settings.
• Experience in customer service is required.
• Experience with managed long-term care insurance is beneficial.
• Minimum of one year of experience working with a frail or elderly population.
• Supervisory experience preferred.
• Must be medically cleared for communicable diseases and have up-to-date immunizations.

Qualifications:

• Current active and unrestricted license and registration in New York County.
• Bilingual skills are preferred.
• Proven ability to work independently and within a team environment.
• Strong communication and interpersonal skills.
• Ability to conduct training and competency sessions with staff.
• Knowledge of care management activities and interventions.
• Fiscally responsible for providing services based on members' needs.
