Medical Social Worker - Per Diem

Duties

• Conduct thorough patient assessments, including psychosocial evaluations and mental health screenings, to identify individual needs and barriers to care
• Develop and implement personalized care plans that incorporate behavioral health strategies, crisis intervention, and social support services
• Provide individual and group counseling sessions focused on mental health counseling, addiction counseling, or behavioral therapy as appropriate
• Collaborate with interdisciplinary teams to coordinate services such as hospice care, senior care, or working with individuals with disabilities or developmental disorders
• Assist with intake procedures, medical documentation, and maintaining accurate medical records in compliance with HIPAA regulations
• Facilitate communication between patients, families, healthcare providers, and community resources to ensure seamless care transitions
• Utilize EMR (Electronic Medical Records) systems for data collection, documentation of clinical notes, CPT coding, ICD coding, and tracking patient progress

Qualifications

• Valid licensure as a Licensed Clinical Social Worker (LCSW) or equivalent in the relevant state or region
• Proven experience in hospital settings, inpatient units, nursing homes, outpatient clinics, or home & community care environments
• Strong knowledge of medical terminology, ICD-10/ICD-9 coding systems, CPT coding, and medical records management
• Experience working with diverse populations including children (pediatrics), seniors (geriatrics), individuals with autism or dementia care needs
• Skilled in crisis management techniques such as PTSD Care and Alzheimer's care strategies
• Familiarity with behavioral health modalities including cognitive behavioral therapy (CBT), motivational interviewing, applied behavior analysis (ABA), and clinical counseling methods
• Ability to handle sensitive situations with professionalism while maintaining confidentiality under HIPAA guidelines
• Excellent communication skills for intake interviews, diagnostic evaluations, case management, and group therapy facilitation
• Knowledge of EMR systems for documentation and data collection purposes

Pay: $32.31 - $38.91 per hour
