Title: Home Care RN Case Manager (Bilingual Spanish)
Location: Danbury, CT
Salary + Benefits: $95,000- $120,000 plus a comprehensive benefits package including Medical, Dental, Vision, PTO, retirement plan, mileage reimbursement, and opportunities for professional growth
Responsibilities of Home Care RN Case Manager:
- Manage a caseload of patients receiving home health services, ensuring individualized and high-quality care
- Conduct initial assessments, develop care plans, and perform regular reassessments in patients’ homes
- Provide skilled nursing care, including wound care, medication management, and health monitoring
- Educate patients and families on disease management, medications, and post-hospital care
- Coordinate care with physicians, therapists, social workers, and other members of the healthcare team
- Monitor patient progress and adjust care plans as needed to achieve optimal outcomes
- Complete timely and accurate documentation in the EMR system
- Ensure compliance with all regulatory and clinical standards
- Provide culturally competent care, effectively communicating with Spanish-speaking patients and families
- Participate in case conferences and interdisciplinary team meetings
Requirements of Home Care RN Case Manager:
- Active Connecticut Registered Nurse (RN) license
- Bilingual in Spanish (required)
- Minimum of 1 year of RN experience; home care or case management experience preferred
- Strong assessment, organizational, and care coordination skills
- Ability to work independently in a field-based setting
- Excellent communication and patient education skills
- Valid driver’s license and reliable transportation
- Proficiency with electronic medical records (EMR) systems
Pay: $95,000.00 - $120,000.00 per year
