We are seeking a compassionate and detail-oriented Social Worker to join our team at a skilled nursing facility. The Social Worker will support residents and their families by addressing psychosocial needs, facilitating care planning, and ensuring a smooth transition throughout the continuum of care. This role plays a key part in enhancing residents’ quality of life and maintaining compliance with regulatory standards.
Key Responsibilities:
- Conduct comprehensive psychosocial assessments for residents upon admission and as needed
- Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team
- Provide counseling and emotional support to residents and families
- Assist with discharge planning, including coordination of community resources and services
- Advocate for residents’ rights and ensure dignity and respect in all interactions
- Facilitate communication between residents, families, and healthcare providers
- Maintain accurate and timely documentation in compliance with state and federal regulations
- Participate in care plan meetings and interdisciplinary team discussions
- Address issues related to adjustment, grief, behavioral concerns, and long-term care placement
- Stay current on healthcare regulations and best practices in long-term care social work
Qualifications:
- Bachelor’s or Master’s degree in Social Work (BSW or MSW) required
- Previous experience in a skilled nursing facility or healthcare setting preferred
- Strong interpersonal, communication, and organizational skills
- Ability to handle sensitive situations with professionalism and empathy
- Knowledge of Medicare, Medicaid, and long-term care regulations is a plus
