Renal Management Inc. is seeking to hire a full-time LMSW/LCSW. Ideal candidate will be professional, competent and have excellent organizational, interpersonal and communication skills. Candidate must have a current Georgia License and a minimum of 1 year of experience working in an outpatient dialysis setting. Candidate must cover two dialysis clinics located in Ellenwood, Georgia and McDonough, Georgia. Both clinics are 3 day a week facilities (MWF).
Responsibilities:
- Case Management
- Conduct ongoing psycho- social assessments and implement appropriate interventions to minimize barriers and promote positive treatment outcomes.
- Support and maximize the social functioning and coping of ESRD patients.
- Advocate for patients
- Provide supportive counseling to patients/families and identify coping strategies related to their illness, modality and lifestyle changes.
- Demonstrate knowledge of local, state and federal regulatory agencies.
- Provide continuing education to patients/families/staff as needed*
- Acts as a liaison between the hospital Social Worker and the patient concerning the patients problem(s) in order to provide continuity of care.
- Participates in administrative and staff programs as required, this includes, but not limited to QAPI, attending staff meetings and in-services.
- Collaborates with staff regarding a change in patients modality and in facilitating a patient transfer, in or out of the facility.
- Collaborated with staff on admission of new patients.
- Gather information about the patients social, psychological, cultural, environmental and financial situation to form treatment plan.
- Coordinates patients funding information with both the administrative office and other agencies to determine eligibility with Medicare, State Medicaid, COBRA and private insurance carriers.
- Knowledge of AKF programs
- Working knowledge of Crownweb
Requirements:
- Master’s Degree in Social Work accredited by the Council on Social Work Education with specialization in clinical practice
- Must possess and maintain a current license to practice in the state of Georgia
- A minimum of 1 year of experience in an outpatient dialysis facility
- Travel between dialysis clinics required
Job Type: Full-time
Pay: $80,000.00 - $85,000.00 per year
