Description

The Bowling Green Veterans Center is a skilled nursing facility that cares for the Commonwealth of Kentucky’s Veterans. Our mission is to Serve Kentucky’s Veterans by fostering independence, preserving dignity and providing compassionate care.

The KY Veterans Ctr Social Work Specialist will provide professional social work and counseling services to our Veteran residents. This position supports the Social Services Section by helping coordinate, evaluate, and implement social services programming while ensuring residents’ psychosocial needs are assessed, addressed, documented, and incorporated into the interdisciplinary care plan.

This position may include, but is not limited to, the following:

  • Assess and document medically-related social service needs, psychosocial histories, and plans of action and outcomes; determine care and treatment services necessary to meet resident needs.
  • Monitor residents’ social and emotional status, adjustment to the facility, and treatment progress.
  • Participate in resident assessments, care planning, and discharge planning, including development and implementation of social care plans.
  • Interview residents, families and support persons, physicians, clergy, and other involved parties to complete psychosocial assessments.
  • Encourage and support resident and family or support person interaction to maintain relationships and support systems.
  • Promote resident participation in social and religious activities as appropriate.
  • Coordinate resident support with family and community resources and support groups to maximize benefit to Veterans.
  • Interpret psychosocial needs for medical staff and other interdisciplinary team members.
  • Assist residents and families in obtaining resources from community agencies.
  • Provide in-service training for employees and others as requested.

EXPERIENCE, TRAINING, OR SKILLS: Two years of professional social work experience.

Substitute EDUCATION for EXPERIENCE: A master's degree in social work will substitute for one year of the required experience.

Substitute EXPERIENCE for EDUCATION: NONE

SPECIAL REQUIREMENTS (AGE, LICENSURE, REGULATION, ETC.): Must be licensed in Kentucky as a social worker by the Kentucky Board of Social Work. Must maintain any required licensure (s), certification(s), or other credentials for the length of employment in this job classification. Employing agency is responsible for ensuring employee possesses and maintains required licensure(s), certification(s) or other credentials.

Incumbents working in this job title typically perform duties in an office setting.

Probationary Period

This job has an initial and promotional probationary period of 6 months, except as provided in KRS 18A.111.
