Job Title: Licensed Social Worker | Home Health Care & Hospice

Rates: Per visit basis
Education/Experience:
- Valid Driver’s License required
- Graduate of Master’s degree program accredited by the Council on Social Work Education.
- Minimum of one year Post Master’s Social Work experience
- Experience in home health, hospice or a health care setting preferred.
Job Summary: providing guidance and support to patients and their families related to the patient's illness and care environment
Job Description:
Assessment of psychosocial needs of patients/families who require assistance with:
- Medical insurance
- Comprehending or responding to instructions necessary to care
- Illness or trauma bringing role reversal
- Severe financial hardship or problems with finances or medical bills
- Long Term planning
Communicates assessment findings with the team
Completes financial review for charity care/sliding scale determination
Provides emotional support to patient/family related to declining status, anticipatory grief, and grief and loss issues
Conducts bereavement risk assessment for Hospice patients
Assists patient/family with securing durable power of attorney, funeral arrangements as needed
Promotes dignity and comfort in end of life care
