Job Overview
We are seeking a dedicated and compassionate Clinical Social Worker/Discharge Planner to join our dynamic healthcare team. Responsibilities
- Conduct thorough psychosocial assessments, including diagnostic evaluations and patient care planning, to identify individual needs and develop tailored discharge strategies.
- Collaborate with medical teams, therapists, and community resources to coordinate safe and efficient patient discharges.
- Provide individual, group, and family counseling utilizing evidence-based approaches such as cognitive behavioral therapy, psychodynamic therapy, motivational interviewing, and behavioral health techniques.
- Manage crisis interventions for patients experiencing acute mental health issues or behavioral crises, ensuring safety and stabilization.
- Maintain accurate medical documentation, including medical records, HIPAA-compliant notes, CPT coding, ICD-9/10 coding, and other essential records for billing and compliance purposes.
- Facilitate communication with patients’ families and caregivers to support understanding of treatment plans and community resources.
- Utilize Electronic Medical Record (EMR) systems for data collection, case management, and clinical research initiatives related to behavioral health outcomes.
Qualifications
- Master’s degree in Social Work (MSW) from an accredited program; licensure as a Licensed Clinical Social Worker (LCSW) or equivalent required.
- Proven experience in hospital settings or home & community care environments with a focus on behavioral health or mental health counseling.
- Strong knowledge of medical terminology, medical documentation standards, HIPAA regulations, and ICD coding systems.
- Expertise in psychotherapy modalities such as cognitive behavioral therapy (CBT), psychodynamic therapy, addiction counseling, PTSD care, Alzheimer’s care, autism spectrum disorder interventions, and working with individuals with disabilities or developmental delays.
- Familiarity with EMR systems for documentation and research purposes; experience with CPT coding is preferred.
- Ability to perform comprehensive patient assessments including intake procedures and diagnostic evaluations across diverse populations including pediatrics and geriatrics.
- Excellent crisis management skills combined with the ability to work effectively with children & families in high-pressure situations.
- Strong interpersonal skills for case management, social work interventions, group therapy facilitation, and collaboration within multidisciplinary teams.
