Job Title: Hospice & Home Health Social Worker
Location: Wahoo, NE (Serving Saunders County)
Job Type: Part Time (24-32 Hours Weekly)

Our interdisciplinary team works collaboratively to support patients’ physical, emotional, and psychosocial needs in the comfort of their homes. We are a small and locally owned Hospice & Home Health agency that prides ourselves on caring for our patients that same that would care for our own family.
Position Summary
We are seeking a dedicated and empathetic Social Worker to join our hospice and home health team. In this role, you will provide psychosocial support, counseling, and resource coordination for patients and their families, helping them navigate complex emotional, social, and practical challenges related to illness, end-of-life care, and recovery.
Key Responsibilities
- Conduct comprehensive psychosocial assessments for patients and families
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Provide emotional support, counseling, and crisis intervention
- Assist patients and families with advance care planning and decision-making
- Connect patients and caregivers with community resources and support services
- Support discharge planning and transitions of care
- Educate patients and families on coping strategies and available services
- Maintain accurate and timely documentation in accordance with regulatory standards
- Coordinator of Volunteer & Chaplin Services
- Coordinator of Bereavement Services
Qualifications
- Bachelor’s or Master’s degree in Social Work (BSW or MSW required; MSW preferred)
- Current state licensure (LMSW, LCSW, or equivalent, as applicable)
- Minimum [1–2] years of experience in healthcare, hospice, or home health preferred
- Strong interpersonal, communication, and counseling skills
- Ability to work independently and as part of an interdisciplinary team
- Knowledge of community resources and healthcare systems
- Valid driver’s license and reliable transportation

Pay: From $28.00 per hour
