• Evening Shift: 3:00 PM – 11:00 PM
• Total Hours: 37.5 per week

Job Summary:
We are seeking a compassionate and detail-oriented Virtual Care Coordinator to support telehealth services within a skilled nursing environment. This role involves coordinating virtual care operations, supporting patients and providers, and ensuring efficient, secure delivery of telehealth services.

Key Responsibilities:

• Perform quality assurance and peer reviews for telehealth and virtual observation programs
• Evaluate program performance and recommend improvements
• Meet with senior leadership to review metrics, feedback, and workflow enhancements
• Lead case reviews with hospital partners and monitor outcomes
• Triage and assign cases for virtual monitoring and observation
• Collaborate on telehealth policies, workflows, and implementation strategies
• Maintain and manage operational databases, including performance metrics, response times, and quality indicators

Qualifications:

• High School Diploma required
• Minimum 2 years of telehealth experience required
• Experience in direct patient care preferred
• Strong computer skills and familiarity with EMR systems
• Excellent organizational and communication skills
• Ability to multitask with strong attention to detail

• Ability to work independently in a fast-paced environment
• Strong problem-solving and coordination skills
