About the Role
Mayfair Care Center is seeking a compassionate and dedicated Social Worker to join our team. In this role, you’ll play a vital part in enhancing residents’ quality of life by addressing their social, emotional, and psychological needs while supporting families throughout their care journey.
Key Responsibilities
Conduct interviews with residents and families to develop personalized care plans
Collaborate with families to promote inclusivity, independence, and well-being
Assist with discharge planning and provide solutions for resident needs, including financial resources
Work closely with interdisciplinary teams to ensure holistic, resident-centered care
Advocate for residents by addressing social and emotional concerns and maintaining clear communication
Requirements
Minimum of 2 years of experience as a Social Worker in a Skilled Nursing Facility (SNF) required
Strong communication and interpersonal skills
Compassionate, organized, and resident-focused
Experience with Sigma EMR system preferred
