Social Worker (LCSW) – Acute Care
Location: Fort Walton Beach, FL
Job Type: Permanent Full-Time
Shift Timings: Day Shift (5x8-Hour) (08:00 AM – 04:00 PM)
Pay: $24.93 – $33.59/hour

Job Description:

• Perform comprehensive biopsychosocial assessments for patients across inpatient units and the emergency department.
• Provide crisis intervention, emotional support, and counseling to patients and families facing acute, chronic, or terminal illnesses.
• Coordinate discharge planning, including post-acute care placement, financial assistance, and community resource referrals.
• Collaborate with interdisciplinary teams including physicians, nurses, and case managers to ensure continuity of care.
• Assist patients and families with complex decision-making while respecting their values and preferences.
• Maintain accurate documentation and ensure compliance with Medicare, Medicaid, and private payer regulations.

Key Responsibilities:

• Work closely with care teams to identify and resolve barriers to discharge (financial, psychosocial, medical, and emotional).
• Conduct high-risk screenings and develop individualized care and discharge plans.
• Facilitate meetings between patients, families, and healthcare teams.

Requirements:

• Master’s Degree in Social Work (MSW) – Required
• Active Florida LCSW License – Required
• Minimum 1 year recent Acute Care Hospital Social Work experience (or 5+ years prior hospital experience)
• Discharge Planning experience – Mandatory

Pay: $24.00 - $33.00 per hour
