Job Overview
We are seeking a dedicated and compassionate PRN (as-needed) Social Worker to join our dynamic healthcare team. In this vital role, you will provide expert social work services to patients across various settings, including inpatient, outpatient, hospice, and community care environments. Your expertise will help improve patient outcomes by addressing psychosocial needs, coordinating care plans, and supporting families through challenging times. Duties
- Conduct comprehensive patient assessments to identify psychosocial needs, including mental health status, family dynamics, and social determinants of health
- Provide individual and group psychotherapy sessions utilizing techniques such as behavioral therapy, motivational interviewing, and cognitive behavioral therapy
- Assist patients with crisis intervention and case management to ensure timely access to resources and support systems
- Collaborate with medical teams to develop and implement personalized care plans that incorporate social work interventions
- Maintain accurate medical documentation in compliance with HIPAA regulations, including detailed notes on patient interactions and treatment progress
- Coordinate discharge planning and post-acute care services for patients transitioning from hospital or nursing home settings
- Collect data for clinical research projects related to behavioral health, addiction counseling, dementia care, or autism spectrum disorders
Requirements
- Valid state licensure as a Licensed Clinical Social Worker (LCSW) or equivalent credential required
- Proven experience in clinical counseling, social work, or behavioral health settings; hospital experience preferred
- Knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and electronic medical records (EMR) systems such as Epic or Cerner
- Strong skills in child & family counseling, hospice care, addiction counseling, and working with individuals with developmental disabilities or autism spectrum disorders
- Ability to perform diagnostic evaluations and utilize evidence-based practices like applied behavior analysis or trauma-informed care
- Excellent communication skills for intake assessments, patient education, and interdisciplinary collaboration
- Experience working with diverse populations including geriatrics, pediatrics, post-acute care patients, and those with dementia or PTSD
- Familiarity with HIPAA regulations and maintaining confidentiality of sensitive medical records
