This is a part-time teleworking position working 30 hours/week with the expectation that you will be visiting clients in the community setting.

  • Bachelor’s degree in a health related/social services field required, Masters preferred.
  • Active certification in case management required, CCM preferred.
  • Experience with providing support/coordination within a health care delivery system including but not limited to public and private insurance options, particularly as it pertains to individuals with complex health care needs and/or disabilities.
  • Experience and demonstrated comfort level with providing advocacy for clients with disabilities.
  • Familiarity with education and rehabilitation systems, particularly as it pertains to individuals with complex health care needs and/or disabilities.
  • Familiarity with home accessibility elements.
  • Ability to work within a team structure, collaborating to achieve efficient and effective client outcomes.
  • Ability to initiate independent follow-up of client needs.

What You Will Do:

This position supports the global needs of private care management clients served by the Medical Legal Services division and enhances its operations and development. It includes the coordination of day-to-day care management services including medical care, rehabilitation therapy, community inclusion, residential services and other needs identified in the client’s Care Management Plan to maintain their optimal health and well-being in the community. The Care Manager practices according to the definition of case management and ethical statements guiding case management practice as determined by The Case Management Society of America and accreditation requirements.

The Care Manager collaborates with other members of the client’s multidisciplinary team and is required to travel throughout the state of Maryland to visit clients in their home, health care facilities, schools, and other residential facilities. This role works in collaboration with the life care planners and other certified case managers (CCM) in the division and in collaboration with the Research and Administrative positions. The position also supports the division's growth by participating in the development of processes and protocols that advance its effectiveness and efficiency.

As a Care Manager, you will:

  • In collaboration with the client and their designated family and/or support personnel and care providers, complete care management assessments and develop an individualized Care Management Plan including the establishment of care management goals.
  • Engage the case management process and best practices to guide client service delivery to include assessment, documentation, planning, implementation, education, advocacy, and evaluation.
  • Coordinate services and support required to facilitate the implementation of the client’s Care Management Plan which includes the location, coordination, and navigation of medical services and other community-based services for medically complex individuals (children and adults).
  • Identify and assist with the resolution of issues, as identified with the client and/or their multi-disciplinary team, that arise in the support of achieving care management goals.
  • Work in collaboration with the client, their family members, their trustee, physicians, therapists, school, community-based programs, other care managers and any other team members and/or individuals involved in the client’s day-to-day care to achieve their goals and those identified in the Care Management Plan.
  • Monitor the provision of on-going services identified in the clients Care Management Plan.
  • Evaluate and adjust the interventions of the Care Management Plan to maximize the client’s health and ability to achieve the goals identified the Care Management Plan.
  • Utilize phone contacts, onsite visits, and participation in multi-disciplinary team meetings to support the achievement of Care Management Plan goals.
  • Locate and coordinate appropriate goods, services, and resources to meet the client's needs and goals, both short and long term.
  • Collaborate with the client and the funder to provide onsite management of each client’s medical and rehabilitation needs as well as provide support for the coordination of residential and transportation needs.
  • Collaborate with community-based specialists and other coordinators of care to implement Care Management Plan goals.
  • Provide advocacy for the client’s needs to assist them with optimizing their access to medical, rehabilitative, and community-based services.
  • Purchase goods and services online and in stores and arranges for the delivery of purchased goods as needed by the client.
  • Establish and maintains effective working relationships with the client, the funder, and all other identified providers.
  • Remain abreast of accreditation, continuing education, and regulatory requirements.
  • Documentation of all interventions with and on behalf of the client in identified software programs. Maintains confidentiality of Personal Health Information in all settings per organizational policy.
  • Support the development of more efficient and effective solutions for division workflows and processes including, as appropriate, technology options for documentation and information tracking.
