Social Work Counselor

Location: Hempstead, NY | Salary: $23.08/Hour | Schedule: Full-time, 35 hours per week

The Opportunity

The Social Work Counselor will provide education and develop independent living skills in foster care children referred by the Department of Social Services.

Responsibilities

• Conduct monthly home visits.
• Assist in developing and facilitating Independent Living Skills Workshops
• Complete a Step Up Independent Living assessment and treatment plan in partnership with client, foster family and caseworker.
• Complete and maintain client case records in compliance with agency and other regulatory standards.
• Complete all required reports, correspondence and other paperwork and submit by requested deadlines.
• Coordinate service efforts with DSS caseworker regarding client's plan
  Provide contacts and supports for the foster families to facilitate the teaching of independent living skills in the home.
• Provide aftercare services to all PI program youth
  Assume other tasks and responsibilities assigned to support the operations of the program

Qualifications

• Bachelors degree in Human Service related field.
• At least one year of experience working with adolescents
• Proficient computer skills required.
• Ability to do home visitation.
• Valid and clean drivers license.
