Responsible for coordinating the care delivered to patients in the acute inpatient rehabilitation setting, including the development of a comprehensive treatment plan for achieving maximal functional independence, coordination of a safe transition to next level of care, and making post-discharge arrangements. Works collaboratively with Medical Director, physicians, therapy staff, nursing staff, payers, Admission Coordinators, external and internal vendors, and community resources so that expected patient and program outcomes such as appropriate service utilization, levels of care and transition arrangements are achieved. Provides psychosocial support and assistance to patients and families such that patients move through the continuum of care in a safe and timely manner; Performs initial inclusive assessment; coordinates and refines individualized discharge plan; assures team management of barriers to discharge; responds to emotional factors; maintains knowledge of and connects patients with appropriate community resources and services; advocates for patients’ rights including self-determination; Facilitates communication among care providers; Works daily with treatment team to maximize patients' time in available therapeutic activities; Documents professional/clinical activities in medical record and in computer systems in a concise and timely manner; adheres to organizational policies and procedures, regulatory accrediting body requirements, and professional practice standards.

- Oversees and faciliates the team conference process, including scheduling to assure regulatory compliance, determines need for and coordinates family meetings, and ensures all related documentation/signatures are completed and in the medical record.
- Develops a case management plan to monitor all admissions for appropriate level of care and ability to participate in extended therapeutic activities. Develops and changes plan as appropriate for a safe and acceptable transition to next level of care, in accordance with patient/family goals. Collaborates and meets with patients and families to implement rehabilitation plans that include behavioral, residential, social, and/or employment goals and serves as a liaison between the patient/family and the treatment team.
- Provides ongoing psychosocial support and assistance to patients and families such that patients move through the continuum of care in a safe and timely manner.
- Works with Bannasch leadership to manage the interdisciplinary team process throughout the patient's stay and continuity of care; works to reduce barriers to interdisciplinary team process and barriers to access of care for continuity of care; Proactively addresses issues within the team and acts to modify behavior to facilitate effective relationships and outcomes.
- Manages ongoing insurance authorization and utilization review requirements post-admission, including managing ongoing insurance authorization process for all managed care patients, completing/providing clinical updates to third party payers as requested, and completing EQHealth required documentation to demonstrate medical necessity and quality of care provided; Responsible for managing and completing all required paperwork to attain retro-Medicaid authorization for unfunded patients post-discharge from IRF.
- Participates in special project completion for unit, which can include leadership opportunities, such as creating positional and unit process standard work, drafting policies and procedures, and building educational resource guides for patients and families; Takes a leadership role in assuring that the unit meets all accreditation (CARF) requirements related to safe and well-supported transitions back to community. Oversees, coordinates, and participates in weekly support groups for patients and famillies. Working with Volunteer Services, oversees and coordinates patient peer support volunteers. Manages the unit's patient and family advisory committee, including member solicitation, meeting schedule coordination, and timely communication of committee feedback and input to IRF leadership.
Essential:
- Excellent organizational and prioritization skills. Good communication and interpersonal skills in dealing with patients, medical and clinical staff, payers, service providers, and community agencies.
- Knowledge of discharge planning, community resources, case management and processes, managed care systems and regulatory requirements.
- Knowledge of various medical disease processes, clinical patient care techniques, equipment and treatment modalities.
- Knowledge of counseling theories and techniques.
- Capable of medical record review, analysis and interpretation of insurance plan benefits, and contracts. Ability to convey medical information in written and verbal formats. Basic computer skills required.
- Knowledge of Insurance criteria for IRF's, Knowledge of TJC, CARF, and IRF criteria Comprehensive knowledge of discharge planning processes, disease related issues, community resources, and issues related to continuity of care.
