Social Worker

Job Overview
We are seeking a dedicated and compassionate Social Worker to join our team, committed to supporting individuals and families through challenging circumstances.

Responsibilities

• The Social Worker will conduct thorough assessments of clients’ situations to identify needs (e.g. employment, housing, participant direction, independent living, justice, foster care).
• Develop individualized care plans.
• The Social Worker will Provide crisis intervention and management during emergency situations to ensure immediate safety and support.
• Collaborate with multidisciplinary teams, including legal professionals specializing in family law, healthcare providers, and community organizations.
• The Social Worker will Maintain accurate documentation of case progress, interventions, and outcomes in compliance with organizational policies and confidentiality requirements.
• Stay informed about current practices in child welfare, behavioral health, and conflict management to continuously enhance service quality.

Requirements

• Bachelor’s degree or higher in social work, psychology, sociology, or a related field; licensure or certification as required by local regulations.
• 2+ years Field case management experience, ideally with Geriatric long-term care patients. (team handles members who are 65+)
• High volume caseload experience
• Bilingual SPANISH
