Job Summary: The Discharge Planner is responsible for coordinating and overseeing individual and group case management services and support provided to individuals struggling with behavioral health issues and addiction.

Core Responsibilities:

  • Provide education on discharge expectations, relapse prevention, and recovery supports
  • Participate in psychoeducational groups as needed.
  • Assist with treatment plans and development of, as needed.
  • Implement AMA blocking strategies.
  • Initiate discharge planning at admission and update throughout the client's stay.
  • Develop individualized discharge plans in collaboration with the client and treatment team.
  • Coordinate step-down placements (e.g., outpatient, IOP, PHP, sober living, recovery housing).
  • Ensure transportation and housing arrangements are confirmed.
  • Facilitate referrals to appropriate levels of care based on ASAM criteria.
  • Communicate with external providers to ensure seamless transitions.
  • Responsible for maintaining a list of referral sources and maintaining relationships with referral sources.
  • Complete timely, accurate, and clinically appropriate discharge summaries.
  • Document discharge plans and progress in the electronic health record (EHR).
  • Assist in the development and implementation of success plans (relapse prevention, coping strategies, recovery supports).
  • Actively participate in multidisciplinary treatment team meetings.
  • Provide updates on discharge barriers and placement status.
  • Collaborate with therapists, medical staff, case managers, and peer supporters.
  • Advocate for client needs and appropriate level of care.
  • Collaborate with probation and parole and provide updates in conjunction with therapy.
  • Assist clients in identifying recovery supports (e.g., sponsors, meetings, community resources).
  • Address barriers such as housing, employment, legal obligations, and transportation.
  • Provide overdose education and resources (e.g., Narcan access if applicable).
  • Attend drug treatment court meetings.
  • Performs other related duties as assigned, consistent with the scope and responsibilities of the position.

Education Requirements: High School Diploma or GED required, bachelor's degree preferred.

License Requirements:

  • CDCA license and QMHS III required.
  • CPR certification required, will train.
  • Must have a valid driver's license, acceptable driving record, be 21 years or older, and be deemed insurable through Team Recovery's insurance carrier.

Experience Requirements:

  • Preferred 3+ years of experience in addiction recovery or behavioral health settings, with a strong understanding of case management and community resource connections.
  • 1 year of Case Management experience preferred.
  • Excellent communication and interpersonal skills, with the ability to collaborate effectively with a multidisciplinary team.
  • Knowledge of evidence-based practices in behavioral health, and a commitment to ongoing professional development.
  • Demonstrated commitment to cultural competence, diversity, equity, and inclusion in clinical practice and program development.
  • Strong analytical skills with the ability to drive innovation and continuous improvement in addiction recovery services.
