As a Care Coordinator, you’ll work side-by-side with our Physicians and Nurse Practitioners in skilled nursing, long-term care, assisted living, and independent living facilities. You’ll play a vital role in ensuring our patients receive the highest quality care.

What You’ll Do:

• Be the bridge between providers, facilities, patients, and families.
• Coordinate patient care and organize census data.
• Pre-round with providers, gathering and presenting key medical information.
• Scribe patient encounters and ensure accurate EMR documentation.
• Track visits and progress notes to maintain consistent care.
• Bill patient visits accurately and on time.
