Requirements of Social Worker(LMSW, LCSW, LCSW-C):

• MD LMSW, LCSW, or LCSW-C
• 2+ years of case management/discharge planning experience required
• Cerner experience preferred

13 weeks, M-F, day shift, no weekends or holidays

Job Description: The Hospital Social Worker is responsible for coordinating and facilitating safe, timely, and effective patient discharges. This role focuses on assessing patients’ psychosocial needs, identifying barriers to discharge, and collaborating with the interdisciplinary care team to develop and implement appropriate discharge plans that ensure continuity of care.
