Responsibilities Include:

• Provide counseling and emotional support to patients and families
• Coordinate community resources and referrals as needed
• Maintain accurate documentation in compliance with regulatory standards

What do I need for this role with Community Care?

• Bachelor's or Master's degree in Social Work (BSW/MSW)
• Current Social Worker license in the state of PA
• Experience in healthcare or related setting preferred
• Strong communication and problem-solving skills
• Driver's License/Vehicle
