Position Overview
We are seeking a compassionate and motivated Rehabilitation Counselor (Case Manager) to support individuals with mental health conditions on their path to recovery. In this role, you will help clients build practical life skills, access community resources, and achieve greater independence and overall well-being.
Key Responsibilities
- Provide direct support and counseling in alignment with individualized psychiatric rehabilitation plans
- Help clients develop essential life skills, including daily living, communication, and vocational skills
- Connect clients to community resources and support participation in social, educational, and recreational activities
- Collaborate with mental health professionals to ensure coordinated and effective care
- Monitor client progress and maintain consistent communication with treatment providers
- Assist in developing and implementing individualized rehabilitation and recovery plans
- Maintain accurate, timely documentation of services, progress, and treatment updates
- Advocate for and empower clients to achieve independence and improve overall well-being
Education & Experience Requirements
Candidates must meet one of the following (in alignment with Maryland PRP guidelines):
- Bachelor's degree in a related field: Social Work, Psychology, Counseling, Sociology, Human Services
- Associate degree +* at least 2 years of experience in behavioral health OR*
- High school diploma/GED + at least 4 years of experience in:
- Mental health services
- Case management
- Residential programs
- Youth or adult support services
Reporting Structure
This position reports to the Rehabilitation Specialist and works closely with the Clinical Director, Program Director, and leadership team.
