Build person-centered behavior plans that move people from crisis toward lasting community stability.
- Bridge hospital to home
Partner with hospitals and CTI teams to navigate the critical transition from inpatient care to the community.
- Lead with expertise
Conduct FBAs, administer psychological assessments, train staff, and consult across complex systems.
What you'll do
- Design and evaluate trauma-informed, strengths-based behavior support plans
- Conduct functional behavioral assessments and interpret standardized tools (WAIS, Vineland, etc.)
- Train/coach direct care staff in proactive behavioral strategies and de-escalation
- Participate in on-call crisis response across the ESD program
- Collaborate across OMH, OPWDD, community systems to unlock access to care
- Carry a specialized caseload of people with complex behavioral needs
You're a fit if you have
- Master's in social work, psychology, counseling, or related field
- Experience with MH/IDD populations (co-occurring a big plus)
- LCSW, LMHC, LMSW or pursuing licensure
- Knowledge of Positive Behavior Supports and recovery-oriented practice
- Familiarity with OMH/OPWDD systems and regulations
- Valid driver's license and flexibility for on-call responsibilities
