Build person-centered behavior plans that move people from crisis toward lasting community stability.

• Bridge hospital to home

Partner with hospitals and CTI teams to navigate the critical transition from inpatient care to the community.

• Lead with expertise

Conduct FBAs, administer psychological assessments, train staff, and consult across complex systems.

What you'll do

• Design and evaluate trauma-informed, strengths-based behavior support plans
• Conduct functional behavioral assessments and interpret standardized tools (WAIS, Vineland, etc.)
• Train/coach direct care staff in proactive behavioral strategies and de-escalation
• Participate in on-call crisis response across the ESD program
• Collaborate across OMH, OPWDD, community systems to unlock access to care
• Carry a specialized caseload of people with complex behavioral needs

You're a fit if you have

• Master's in social work, psychology, counseling, or related field
• Experience with MH/IDD populations (co-occurring a big plus)
• LCSW, LMHC, LMSW or pursuing licensure
• Knowledge of Positive Behavior Supports and recovery-oriented practice
• Familiarity with OMH/OPWDD systems and regulations
• Valid driver's license and flexibility for on-call responsibilities
