Summary
Rensselaer Care Center is seeking a compassionate and organized Dementia Care Unit Coordinator to join our dedicated team at our facility in Rensselaer, IN. In this role, you will oversee the care and well-being of residents with dementia, ensuring they receive personalized, respectful support aligned with our commitment to quality care. Your efforts will directly contribute to enhancing the lives of our residents and supporting their families.

Qualifications

  • Associate’s or Bachelor’s degree in healthcare, social services, or mental health required.
  • Specialized training in memory care or dementia care preferred.
  • Experience working with residents with Alzheimer’s disease or other forms of dementia strongly preferred.
  • Strong communication, organization, and leadership skills.
  • Compassionate, patient, and resident-focused approach to care.
  • Proven experience in dementia or senior care settings.
  • Strong leadership and team management skills.
  • Knowledge of healthcare regulations related to elder care.
  • Ability to develop and oversee individualized care plans.
  • Compassionate approach with a focus on resident dignity.

Responsibilities
Coordinate daily operations of the dementia care unit to ensure a safe, caring environment.

  • Collaborate with healthcare professionals to develop and implement individualized care plans.
  • Supervise staff to ensure adherence to care protocols and company policies.
  • Provide training and support to staff on dementia-specific care practices.
  • Monitor resident progress and adjust care strategies as needed.
  • Maintain accurate documentation in compliance with healthcare regulations.
  • Communicate effectively with residents’ families regarding their loved ones’ care.
  • Promote a compassionate and respectful atmosphere within the unit.'
