Job Title: Virtual Care Coordinator
Location: New York, NY
Duration: 13 Weeks
Schedule: 37.5 hours/week (5x7.5-hour shifts)
Job Summary:
The Virtual Care Coordinator supports telehealth and remote patient monitoring services within a virtual care environment. This role ensures safe, efficient, and high-quality patient care while coordinating between clinical teams, leadership, and technology platforms.
Key Responsibilities:
- Monitor virtual patient care activities to ensure patient safety and clinical effectiveness
- Collaborate on development and implementation of telehealth workflows, policies, and best practices
- Conduct quality assurance reviews and analyze performance metrics for virtual care programs
- Provide real-time technical and clinical support for telehealth and remote monitoring systems
- Monitor patient status and escalate clinical concerns to appropriate care teams
- Coordinate telehealth consults, follow-ups, and remote monitoring assignments
- Train staff on telehealth workflows, systems, and documentation standards
- Maintain accurate documentation of patient observations, interventions, and safety events
- Serve as liaison between clinical teams, IT, and telehealth vendors
- Support reporting, program development, and continuous quality improvement initiatives
Required Experience:
- Minimum 3 years of direct patient care experience
- Minimum 2 years of telehealth or remote patient monitoring experience
- Experience with EMR systems (Epic preferred)
- Strong knowledge of HIPAA compliance and patient privacy standards
Required Skills:

- Ability to work in a fast-paced virtual care environment
Education (Preferred):
- Associate or Bachelor’s degree in a health-related field
