Position Overview

The Remote Support Specialist is the primary point of connection for Journey of Care members, providing scheduled outreach and on-demand support to ensure clients are safe, supported, and never alone. This role serves as an extension of the member’s family — offering proactive monitoring, compassionate engagement, and early identification of risks to prevent crises.

Remote Support Specialists support Medicaid clients by monitoring safety technology, recognizing physical and behavioral health changes, and addressing social, environmental, or financial stressors before they escalate. This position combines relationship-based care
with technology-enabled monitoring, resource coordination, and thorough documentation.

Key Responsibilities

Member Outreach & Support

  • Conduct scheduled outreach calls to assess wellbeing and provide ongoing support.
  • Be available for on-demand calls to address immediate needs or concerns.
  • Build trusting relationships through compassionate, person-centered engagement.
  • Serve as an extension of the member’s support system so they feel connected and cared for.

Safety Monitoring & Risk Prevention

  • Monitor safety technologies (e.g., remote sensors, health monitoring devices, and safety alerts).
  • Follow all safety protocols and defined escalation procedures.
  • Respond promptly to alerts and coordinate appropriate interventions.
  • Complete incident reporting according to organizational guidelines.

Resource Coordination & Home Safety

  • Provide resources and connections needed for members to remain safe in their homes.
  • Coordinate with family members, caregivers, and medical providers when appropriate.
  • Support members in accessing community services, medical care, and social supports.
  • Ensure all communications follow HIPAA and release-of-information standards.

Early Risk Identification

  • Observe and document signs of physical decline, behavioral health changes, social isolation, financial stressors, and environmental safety risks.
  • Escalate concerns according to protocol to prevent crises.

Care Coordination & Advocacy

  • Support members in navigating life challenges and accessing appropriate services.
  • Communicate with care teams, case managers, and families as authorized.
  • Advocate for member needs while promoting dignity, independence, and safety.

Documentation & Compliance

  • Complete accurate, timely documentation of all interactions, alerts, and interventions.
  • Maintain compliance with Medicaid requirements and organizational policies.
  • Identify and report risk factors using established workflows.

Required Qualifications

  • Minimum 1 year of medical experience in an office or healthcare setting.
  • Current student in nursing, social work, or a related healthcare field.
  • Basic understanding of Medicaid and Medicare benefits.
  • Strong communication, documentation, and organizational skills.
  • Compassionate, patient-centered approach.
  • Ability to follow safety protocols, escalation procedures, and incident reporting requirements.
  • Comfort using tablets and remote monitoring technology.

Preferred Qualifications

  • Experience working with individuals with disabilities.
  • Previous case management or care coordination experience.
  • Student pursuing medical licensure or related clinical credential.
  • Experience supporting Medicaid populations, older adults, or individuals with complex needs.
  • Bilingual or multilingual abilities.

Work Environment & Schedule

  • Remote position using a tablet and monitoring systems.
  • Part-time hours initially, with opportunity to grow into a full-time role as the program expands.
  • Flexible scheduling may be required to support member needs.

Pay: $21.00 - $25.00 per hour
