RN Care Coordinator - HIV & Homeless
Assignment: Temp through December (Covering mat leave that starts in July but need someone asap to train)
*Potential for perm role as bridge nurse case manager (following up w/ discharged patients/medically assisted treatments)
Schedule: 24HPW; Tues/Weds/Thurs 8am-4:30pm
Responsibilities:
- Conducts an intake on all new patients which includes assessing the patients understanding of HIV, past medical history, and ART history
- Implements a culturally competent plan of care that will help the client achieve durable viral suppression and evaluates the plan at least yearly
- Provides initial and ongoing education on HIV transmission, prevention, risk reduction, disease progression, and treatment strategies
- Recognizes, assesses, and takes immediate action in acute and crisis situations, both patient and environmental
- Safely administers and accurately documents immunizations and medications
- Safely performs nursing procedures, including phlebotomy and IV insertion after training and validation and transports specimens to laboratory
- Provides wound care as requested by clinician and documents assessment and plan clearly in the medical record
Qualifications:
- Active MA RN License
- CPR/BLS Certification
- Minimum 2 years’ experience in case management or care coordination preferred
- Experience with chronic disease or wound care preferred
Pay: $48.00 - $50.00 per hour
