Job Title: Greyhound PSH Case ManagerLocation: Denver, CO (Greyhound Site)Status: Full-Time, Non-ExemptNote: Unless they hold a clinical license (LCSW/LPC) and meet the "Professional" exemption criteria.Salary Range: $58,000 – $68,000 per year ($27.88 – $32.69/hr)Position SummaryThe PSH Case Manager provides intensive, participant-led services to 50 residents within the Greyhound permanent supportive housing community. This role is strictly service-oriented; you will act as a bridge between residents and community resources while helping them maintain housing stability. You will not handle rent collection, leasing, or building maintenance, allowing for a pure clinical and advocacy-based relationship with residents.Essential Duties & ResponsibilitiesCase Coordination: Maintain a caseload of up to 50 residents, providing individualized support plans focused on housing retention, health, and wellness.Housing Navigation & Stability: Assist residents in understanding their lease obligations and navigating conflicts with property management to prevent evictions.Medicaid-Reimbursable Services: Provide and document "Tenancy Sustaining Services" (H2014) and "Targeted Case Management" (T1017) in accordance with Colorado Health First (Medicaid) standards.Documentation: Maintain high-quality clinical notes that meet the "medical necessity" requirements for Medicaid billing and HUD reporting.Resource Connection: Facilitate referrals for mental health, substance use treatment, primary care, and disability benefits (SSI/SSDI).Crisis Intervention: Provide de-escalation and crisis support within a Trauma-Informed Care framework.QualificationsRequiredEducation: Bachelor's degree in Social Work, Psychology, or a related Human Services field.Experience: Minimum 2 years of experience working with individuals experiencing chronic homelessness or dual diagnoses (mental health and substance use).Medicaid Literacy: Demonstrated ability to document services for Medicaid billing (specifically Colorado Fee-for-Service or MCE models).Values: Deep commitment to the Housing First and Harm Reduction philosophies.PreferredCertification: Qualified Behavioral Health Professional (QBHP) status in Colorado.System Knowledge: Proficiency in HMIS (Homeless Management Information System) and Medicaid billing portals (e.g., Gainwell).Specialized Training: SOAR certification (SSI/SSDI Outreach, Access, and Recovery).The Empowerment Program is proud to be an equal opportunity workplace.
