Home Care Coordinator - LPN

Position Summary: Work closely with the interdisciplinary team at hospitals, skilled nursing facilities/rehabilitation centers and provider offices to facilitate and support home discharges for referred patients through facility staff communications; together with providing support and facilitating the development of safe discharge plans which may include professionals and paraprofessionals, supplies, and equipment delivered in the home setting.


Schedule: Monday - Friday, Days 8 to 430


Qualifications

  • Education: Current active NY State LPN License in good standing.
  • Experience: Minimum of one year of community health nursing in a CHHA preferred.
  • One year of recent clinical experience

Responsibilities

Qualified individuals must be capable of performing the following essential duties and responsibilities, with or without reasonable accommodations:


Care Management:

  • Complete referrals on hospitalized patients for referral to URMHC.
  • Gather data regarding patient’s appropriateness for home care including information of safety, support systems, identified caregivers to learn/perform care, patient willingness/ability to perform care, and payer/financial issues.
  • Possesses and utilizes knowledge of agency and community resources to complete appropriate referrals for services, supplies, and equipment and arrange for timely delivery of such.
  • Identify the home care focus of care.
  • Involves patient and family support system in the referral process to assure maximum involvement in the patient care needs at home.
  • Communicates and coordinates referral process with the interdisciplinary team at UR Medicine Home Care and at institutions as well as with patients including documentation per institution and UR Medicine Home Care standards.
  • Actively participates in marketing UR Medicine Home Care services to referral sources ongoing.
  • Practices standard precautions per UR Medicine Home Care and institution standards including identification of patients for UR Medicine Home Care infection surveillance monitoring and those requiring special precautions in the home.
  • Collaborates with a corresponding RN in an ongoing basis.

Financial Management:

  • Plans referrals with attention to reimbursement issues and efficient use of resources as well as taking responsibility for own productivity.
  • Obtains payer information for all referrals.
  • Initiates assistance of hospital and UR Medicine Home Care patient financial services for patients without insurance.
  • Utilizes institution and UR Medicine Home Care consultants to assure maximum efficiency in care delivery through use of new/improved technology and products.
  • Communicates with patient and family the need to identify able and willing caregivers to be involved in learning and providing care to maximize efficiency in care delivery.
  • Maintain attendance within UR Medicine Home Care standards.
  • Maintain productivity in generating referrals as well as assisting others as needed to limit use of overtime and supplement help.
  • Participates in marketing and identification of new opportunities to grow referral volumes.

Compliance:

  • Complies with agency and regulatory standards.
  • Completes required/mandatory in-services, professional licensing requirements, and employer and institutional requirements.
  • Answers calls in a timely manner.
  • Continuously reads and appropriately responds to emails when working.
  • Adheres to agency standards for documentation of referrals.
  • Complies with Corporate Compliance standards including confidentiality and HIPAA requirements.
  • Complies with safe work practices (ergonomics, OSHA)
  • Submits timely, accurate, complete referrals including communication with Intake and Scheduling as workflow requires. Work site as assigned by manager, including facility or office-based locations.

Hourly Range: $18.33 - $38.90
