About the Role
Are you a compassionate advocate with a heart for end-of-life care? We are seeking dedicated Licensed Master Social Workers (LMSW) to join our San Antonio team.
In this role, you serve as a vital pillar of support for patients and their families during their most vulnerable moments. Whether you’re looking to supplement your current practice or enjoy the flexibility of a per-diem schedule, you will play a crucial role in ensuring no one faces the end-of-life journey alone.
What You’ll Do:
- Conduct home and nursing facility visits.
- Complete admission and pre-bereavement assessments.
- Participate in required Interdisciplinary Team (IDT) meetings.
- Assess psycho-social functioning and needs of patients and family members.
- Develop and implement individualized treatment and care plans.
- Make appropriate referrals to community resources and other agencies.
- Coordinate care and services with patients, families, and healthcare professionals.
Qualifications
- Master’s degree in Social Work.
- LMSW required.
- Minimum of 1 years of hospice experience.
- Strong ability to conduct independent psycho-social assessments and provide treatment.
- Excellent communication, documentation, and organizational skills.
- Reliable transportation and valid driver’s license.
- Current CPR certification.
