Social Worker – Travel Contract

Location: Bristol, CT

Contract Length: 13 Weeks

Guaranteed Hours: 40 per week

Pay Package

Weekly Gross: $2,406

Regular Rate: $22/hr

OT Rate (1.5x): $52/hr

Weekly Non-Taxable Stipend: $1,526

(70-mile radius rule applies)

Unit Details

Shift: 5x8s – Monday through Friday

Hours: 8:00 AM – 4:30 PM

EMR: MediTech

Requirements

• LMSW accepted; LCSW preferred
• Minimum 2 years healthcare setting experience required
• Active CT license
• Hospital-based case management / discharge planning experience preferred

Position Overview

Seeking an experienced Social Worker to support patients in a hospital setting with discharge planning, psychosocial assessments, care coordination, and community resource referrals. Candidate must be comfortable working collaboratively with interdisciplinary teams and navigating complex patient care needs.
