Experteer Overview As a Case Manager at YNHHS, you will coordinate high-value, patient-centered care within a multidisciplinary team. You ensure care is efficient, evidence-based, and aligned with both clinical and financial objectives, spanning inpatient to outpatient pathways. You’ll identify barriers, guide discharge planning, and link patients to resources while advocating for appropriate services. This role offers impact by improving care transitions, reducing variability in practice, and supporting hospital-wide goals. You will work with cross-functional partners to optimize the patient care continuum and resource use. Compensation / Benefits • Coordinate and ensure plan of care implementation within the principles of case management • Establish a care coordination system linking inpatient and outpatient services • Identify and monitor variances from clinical pathway parameters • Utilize resources to meet admission, treatment, and discharge needs and coordinate follow-through • Assist clinicians in documenting admission appropriateness and ongoing stays • Manage Medicare notices of non-coverage and assist with documentation for denials appeals • Develop and implement a discharge plan with the care team, identifying options for services and funding • Ensure continuity of care with outpatient physicians and needed services • Contact and refer to appropriate outside agencies and follow through on referrals • Link patients and families to institutional or community resources, advocate for scarce resources, and develop new resources where gaps exist • Collaborate with PSM and unit leadership to involve nurses in discharge assessment and planning • Act as a patient advocate and address ethical/legal aspects to minimize risk Tasks • RN Licensure active in Connecticut • Minimum of three years of relevant clinical experience • Advanced nursing knowledge, case management, and continuity of care • Strong communication and interpersonal skills • Ability to assess information for patient needs and age • Advanced assessment and teaching skills • Leadership in planning and managing patient care in acute care • Utilization management and case management experience preferred • Knowledge of computer software and basic statistics Key requirements •
