Position Overview
We are seeking a motivated and compassionate Case Manager/Counselor to join our clinical team. In this role, you will manage a caseload of clients, support their treatment journey, and collaborate closely with the multidisciplinary team to ensure high-quality care and positive outcomes.

Key Responsibilities

• Manage a caseload of clients, coordinating care and supporting treatment goals
• Conduct client intakes, admissions, and orientations
• Develop, implement, and maintain individualized treatment plans and progress notes
• Facilitate individual and group counseling sessions
• Monitor client progress and document behavioral changes
• Assist clients with outside obligations (legal, employment, medical leave, etc.)
• Conduct urine drug screens and support medication monitoring as needed
• Coordinate referrals to internal and external resources
• Provide transportation and support for client-related needs when necessary
• Identify crisis situations and respond appropriately
• Communicate regularly with the Program Director and treatment team regarding client progress
• Maintain professional boundaries, ethics, and strict confidentiality

These responsibilities reflect core duties such as case management, documentation, group facilitation, and coordination of care within a structured treatment program .

Qualifications

• Certification or registration to provide substance use disorder (SUD) counseling (required)
• Valid California Driver’s License
• CPR/First Aid certification (or ability to obtain upon hire)
• TB test and health screening required
• Experience in substance abuse treatment, behavioral health, or related field preferred
• Strong documentation, communication, and organizational skills
• Ability to work effectively with diverse populations
• Ability to assess client needs, identify crises, and act appropriately

Experience:

• Substance Abuse Treatment: 1 year (Preferred)

License/Certification:

• CADC, RADT, SUDRC, etc. (Required)

Work Location: In person
