Position Overview
We are seeking a motivated and compassionate Case Manager/Counselor to join our clinical team. In this role, you will manage a caseload of clients, support their treatment journey, and collaborate closely with the multidisciplinary team to ensure high-quality care and positive outcomes.
Key Responsibilities
- Manage a caseload of clients, coordinating care and supporting treatment goals
- Conduct client intakes, admissions, and orientations
- Develop, implement, and maintain individualized treatment plans and progress notes
- Facilitate individual and group counseling sessions
- Monitor client progress and document behavioral changes
- Assist clients with outside obligations (legal, employment, medical leave, etc.)
- Conduct urine drug screens and support medication monitoring as needed
- Coordinate referrals to internal and external resources
- Provide transportation and support for client-related needs when necessary
- Identify crisis situations and respond appropriately
- Communicate regularly with the Program Director and treatment team regarding client progress
- Maintain professional boundaries, ethics, and strict confidentiality
These responsibilities reflect core duties such as case management, documentation, group facilitation, and coordination of care within a structured treatment program .
Qualifications
- Certification or registration to provide substance use disorder (SUD) counseling (required)
- Valid California Driver’s License
- CPR/First Aid certification (or ability to obtain upon hire)
- TB test and health screening required
- Experience in substance abuse treatment, behavioral health, or related field preferred
- Strong documentation, communication, and organizational skills
- Ability to work effectively with diverse populations
- Ability to assess client needs, identify crises, and act appropriately
