Our client, a reuputable Non- Profits Social Services agency is seeking dedicated and compassionate Master of Social Work (MSW) professionals with at least two years of relevant experience to join their dynamic healthcare team. Responsibilities
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Establish and maintain all provider service agreements with appropriate hospitals, clinics, and any other in/out patient services as deemed necessary.
Establish support and social service linkages to deliver appropriate services to residents as deemed necessary, i.e.: individual/group counseling, pastoral services, etc.
Provide site coverage in the absence of the Program Director
Provide monthly supervision to Case Managers
Identify new untapped community resources.
Supervise rehousing/daily living specialists with the development of individualized resident service plans.
Directly engage in the recruitment, supervision and performance evaluations of all clinical team members.
Ensure that all federal, state and local mandates relative to resident care and clinical services are being met.
Provide residence staff with relevant education and training.
Facilitate individual and group counseling to residents.
Oversee the program admission process; including referral, acceptance/rejection, resident move in and development of case plan.
Maintain good working relationship with the HASA Case Management Unit and Housing Unit.
Supervise the timely development of all resident individualized service plans inclusive of medical condition, social supports as authorized by the physician.
Assume liaison functions between the agency and community service provider.
Actively participate in the admission and discharge planning for all residents.
Provide statistics for the monthly project progress notes to include resident movement, diagnosis, prognosis, census and related clinical services that were provided.
Participate in mandated training programs.
QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Master's Degree in social work and two (2) years of experience directed to medically involved individuals. Knowledge of HIV/AIDS. - Minimum of two years of professional experience in social work or mental health counseling within clinical or hospital settings.
- Proven expertise in child & family counseling, hospice care, addiction counseling, or working with individuals with disabilities or developmental disorders such as autism or Down syndrome.
- Experience working with diverse populations including geriatrics, pediatrics, and patients requiring specialized care such as Alzheimer’s or dementia care.
- Familiarity with medical terminology, ICD coding (ICD-9/10), CPT coding, and medical records management.
- Hands-on experience using EMR systems for documentation and data collection purposes.
- Strong background in crisis management, behavioral health interventions, and working within outpatient or inpatient environments.
- Knowledge of HIPAA compliance standards and medical documentation protocols essential for maintaining patient privacy and legal standards. Pay: $70,000.00 - $75,000.00 per year
