The hours are from 08:30 to 17:00. No on-call or weekend requirements, though weekend shifts may be picked up if desired. The role involves floating throughout patient care units as needed. Local candidates must reside within a 60-mile radius. Candidates must possess a valid LMSW license from the applicable state. The role requires BLS certification. Experience in biopsychosocial assessment is necessary. Assist patients in navigating the discharge planning process. Collaborate with care coordination and home care services following discharge. Knowledge of community programs such as food assistance and Medicaid is essential. Familiarity with protective services and guardianship matters is required. All employees must receive flu vaccinations. This position offers an opportunity to work with adult and geriatric patient populations. Daily census will consist of approximately 24 patients. Candidates should be prepared to participate in comprehensive care coordination activities. Two supervisory references are required for this position.
