Vibrant and growing clinic in Hagerstown, Maryland, is seeking Case Manager/Community Support Specialist. Candidates must have bachelor's or master's degree in psychology or a human services field. Experience in a related field is a plus. Incumbent must be compassionate, well-spoken, and show understanding of patient's case management needs. Must be able to work independently and as an effective member of a team of providers and to ensure patients' basic needs, procuring entitlements, medication management, housing, and other tasks. Must pass our required background check. Must utilize scheduling, electronic medical record, and utilize other programs as needed. Pay rate depends on education and experience of applicant.
