We are seeking dedicated Licensed Master Social Workers (LMSW) to join a growing behavioral health / mental health team. This hybrid role is ideal for professionals experienced in clinical social work, case management, and patient care coordination.

• Conduct psychosocial assessments and care planning
• Provide mental health counseling & case management
• Collaborate with interdisciplinary healthcare teams
• Maintain clinical documentation (EMR/EHR systems)
• Ensure compliance with Medicaid/Medicare guidelines

• Active New York LMSW license
• 1+ year experience in clinical social work / behavioral health / mental health
• Medical Spanish (mandatory)
• Experience with Medicaid/Medicare patients preferred
• Must be based in the Tri-State area
