What You’ll Do:

• Assess and support the social and emotional needs of residents and families
• Develop individualized care plans with the interdisciplinary team
• Provide one-on-one support and counseling
• Lead education sessions and connect residents with resources
• Play an active role in enhancing quality of life across the community

What You’ll Bring:

• Bachelor’s Degree in Social Work (preferred)
• Licensed Social Worker (LSW) in Ohio – required
• Experience with mature adults preferred
• Empathy, professionalism, and a strong sense of purpose
