Position Overview: The Care Manager is responsible for overseeing and coordinating comprehensive care services for members. This role involves enrollment activities, assessment and care planning, advocacy, and support in accessing public benefits, ensuring continuity of care, and documenting all services provided. This is a full-time position requiring a 40-hour work week, with potential travel within selective boroughs of NYC, including the Bronx, Manhattan, Brooklyn, Queens, and Staten Island, and Westchester County

Key Responsibilities:

  • Advocacy: Educate members, caregivers, and support networks about the member’s diagnosis, strengths, needs, and goals, serving as an advocate across institutional settings, homes, and community locations. Assist members in accessing and maintaining public benefits, including Medicaid, Cash Assistance, Social Security, Supplemental Nutrition Assistance Program (SNAP), housing, legal services, and employment resources.
  • Care Coordination: Conduct thorough assessments of each member’s needs to develop and manage individualized Plans of Care (POC) that align with their personal goals. Collaborate closely with medical and mental health providers and community resources to provide comprehensive care, ensuring that all aspects of the member’s well-being are addressed effectively.
  • Linkage to care: Assist with referrals to additional services and resources while assisting members in following through with appointments to access necessary services. Connect members with community care options and support networks, reinforcing a continuum of care and ensuring their specific needs are met.
  • Support: Provide ongoing assistance to Medicaid recipients with chronic medical and mental health conditions to navigate available resources and ensure their overall well-being. Conduct home and field visits as needed to engage with members and their families directly, addressing their needs and concerns while fostering a supportive relationship.
  • Documentation and Compliance: Meet all monthly billing requirements and documentation standards set by DOH and Lead Health Home. Maintain accurate case records and document all services provided, ensuring that the member and their family receive the necessary support.
  • Training and Development: Attend regular supervision sessions with a direct supervisor for guidance and support. Participate in mandatory training and annual DOH training, along with additional organization-wide training and refresher as required. Perform additional duties as assigned.

Requirements:

  • Bachelor’s degree is required and two years of relevant experience working with populations facing complex medical, mental health, and psychosocial needs (experience with children and adolescents is a plus)
  • Proficiency with modern communication tools, including computer usage, Office 365, and Healthcare Management systems, is essential.
  • Bilingual candidates are preferred- Must also speak English (various languages)- Russian, Spanish, Bengali, Hindi, Urdu, French, Haitian, Arabic, Mandarin, Cantonese

** Starting pay is $26/hr **
