Position Summary

We’re seeking a compassionate and motivated Medical Social Worker (MSW) to join our Home Health team on a PRN basis in the Dublin, Ohio area.
The MSW will evaluate and support patients and families dealing with medical, emotional, and social challenges — helping them access the resources and support needed for successful care at home.

Key Responsibilities

✅ Provide psychosocial evaluation, diagnosis, and treatment planning
✅ Offer individual, family, and/or group therapy as needed
✅ Assess and address social or personal issues impacting patient independence and recovery
✅ Connect patients and families to appropriate community resources
✅ Collaborate with physicians, nurses, and interdisciplinary care teams
✅ Maintain accurate, up-to-date documentation per HIPAA standards
✅ Provide written reports for patient records, including evaluations, progress notes, and discharge summaries
✅ Participate in family conferences and patient care planning
✅ Discuss care concerns and recommendations with the Director of Clinical Services

Qualifications

✔ Master’s degree in Medical Social Work (MSW) or equivalent license
✔ Minimum 2 years of experience in a medical or home health setting
✔ Current CPR certification
✔ Valid driver’s license and proof of auto insurance
✔ Ability to attend a 1–2 day orientation at our Dublin office
