Children's Home Society of WV is seeking a full-time Regional Care Coordinator to coordinate and supervise the Right From the Start program and staff at a regional level. This position provides direction to and monitors agencies providing services to clients in accordance with project standards and protocols. While there is no on-call responsibility with this position, a considerable amount of travel is required.

Right From The Start (RFTS) is a statewide program that helps West Virginia mothers and their babies lead healthier lives by offering home visitation services with a Designated Care Coordinator (registered nurse or licensed social worker). Services are provided at no cost and support mothers, their new babies and their families by helping create a safe, nurturing home. Designated Care Coordinators discuss any questions and concerns the client may have and help find needed resources.

What you will be doing:

  • Comply with Right From the Start (RFTS) program guidelines to administer a regional system for eligible pregnant women and infants.
  • Direct the day-to-day operation of RFTS program at the regional level.
  • Provide project administration, coordination, direction, and training to ensure that project goals and objectives are identified, met, and evaluated.
  • Supervise program staff and ensure efficiency.
  • Assess and refer clients to a Designated Care Coordinator in a timely manner.
  • Recruit, provide direction for, and negotiate appropriate agreements with identified individuals/agencies to ensure that eligible clients can access medical care, health education, and nutrition services providing an array of choices for clients to choose from.
  • Provide education, consultation, and support to medical providers and other professionals to facilitate delivery of timely services to eligible clients.
  • Provide education, consultation, and support for individuals or agencies who provide direct RFTS services.
  • Participate in community outreach activities as means of promoting the program and engaging with other service providers.
  • Maintain information systems for risk assessment and client tracking.
  • Track and review client records to ensure appropriate care throughout client involvement with the program and at discharge.
  • Ensure the timeliness, accuracy, and grammatical construction of all reports and correspondence.
  • Work cooperatively with the WV Department of Health and Human Resources' Office of Maternal, Child, and Family Health to ensure the smooth operation of the RFTS program.
  • Ensure that all activities and services are conducted in a manner that is sensitive to and shows respect for the culture and ethnicity of the families and children the agency serves.

What you will need:

  • Master's degree in Social Work or RN with current social work or nursing license required.
  • At least 3 years experience in obstetrics and infant nursing or social work preferred.
  • Previous supervision and administrative experience preferred.
  • Basic computer and data entry skills.
  • Valid driver’s license and automobile insurance coverage.
  • Criminal Background Check and Child Abuse Registry Check.
  • Ability to understand and support the values of family-centered services, which include committing to family stability and development of skill in assessing risk to children to ensure safety.
  • Willingness to commit to the vision of permanency for every child.
