We are seeking a Behavioral Health Care Manager (BHCM) to join our pediatric practice in Cary, NC as part of an innovative Collaborative Care Model (CoCM) integrating behavioral health into primary care.

The BHCM works closely with pediatricians and a consulting psychiatrist to support children and adolescents experiencing mild to moderate behavioral health conditions such as ADHD, anxiety, and depression. This role provides brief, evidence-based behavioral health interventions, symptom monitoring, and care coordination within a structured team-based model.

This is an excellent opportunity for clinicians who value interdisciplinary collaboration, measurement-based care, and predictable work hours.

Key Responsibilities

  • Conduct behavioral health screenings and assessments following pediatric referral
  • Administer validated screening tools (PHQ-9, GAD-7, Vanderbilt, etc.)
  • Provide brief evidence-based interventions including problem-solving, motivational interviewing, behavioral activation, and CBT-informed strategies
  • Track patient progress using measurement-based care tools
  • Participate in weekly psychiatric case consultation
  • Provide psychoeducation and support to patients and families
  • Coordinate referrals to community behavioral health providers when longer-term care is needed
  • Document care in the electronic health record and patient-tracking registry

Qualifications

  • Master’s degree in Social Work, Counseling, Psychology, Marriage & Family Therapy, or related field
  • Active North Carolina licensure (LCSW/LCSW-A, LCMHC/LCMHC-A, LMFT/LMFT-A)
  • Minimum 1-2 years post-graduate experience working with children and adolescents
  • Strong collaboration and communication skills
